PA Department of Health requirement for authorization for select infant formulas

Abbott, the makers of Similac formulas, has adjusted the caloric density of some of its infant formulas to align more closely with that of human milk. These infant formulas include Similac Sensitive, Similac Total Comfort, and Similac for Spit Up.

 This calorie change differs slightly from the US Department of Agriculture’s requirements; therefore, USDA is now requesting that medical providers complete a Formula Authorization Form to document the medical need for these formulas for infants who participate with the Supplemental Nutrition Program for Women, Infants and Children (WIC).
