Drug Shortage List (Last Updated 02/29/2016)

The following drugs are not available in the hospital and are not available for reorder from the wholesaler or manufacturer:

 

  • Acetaminophen w/Codeine (Capital w/Codeine) Oral Suspension 120 mg-12 mg/5 mL (alcohol free)
  • Ciprofloxacin 10 mg/mL – 20 & 40 mL vials (No shortage on premixed IV bags)
  • Chloramphenicol 1 g vial
  • Droperidol IV
  • Indigo Carmine 0.8% amps
  • Lidocaine 1 gm/250 mL bags
  • Ofloxacin 0.3% Otic Drops – All sizes
  • Sodium Chloride (Concentrated) 2.5 mEq/mL – 20 mL vial
  • Thrombin 20,000 Pump & Syringe Spray Kits (20,000 unit vials are readily available)

 

The following drugs are available in the hospital in a very limited supply and are not available for reorder from the wholesaler or manufacturer:

 

  • Atropine 0.4 mg/mL – 1 mL vials
  • Caffeine/Sodium Benzoate 500 mg/2 mL vials
  • Fomepizole 1.5 gm/1.5 mL vial
  • Imipenem/Cilastatin (Primaxin) 250 mg vials
  • Pred-G Ophthalmic Ointment – 3.5 gm
  • Thrombin 5,000 unit Epitaxis Spray Kit  (5,000 unit vials other spray kit are readily available) NEW

 

The following drugs are available in the hospital in a very limited supply and small quantities are coming in from the wholesaler or manufacturer:

 

  • Ampicillin/Sulbactam (Unasyn) vials – All strengths
  • Atropine 1 mg/10 mL syringe
  • Calcium chloride 1 gm/10 mL syringe
  • Cefotaxime (Claforan) 1 gm & 2 gm vials
  • Cefotetan – All sizes
  • Dextrose 25% – 10 mL syringe
  • Dextrose 5% IV “Glass” (Non-PVC) bags – 250 mL and 500 mL bags
  • Dextrose 50% – 50 mL syringe
  • Epinephrine 1:10,000 (0.1 mg/mL) – 10 mL syringe
  • Heparin 1,000 units/500 mL premixed bags
  • Heparin 25,000 units/250 mL premixed bags
  • Lidocaine 100 mg/5 mL syringe
  • Pipercillin/Tazobactam (Zosyn) – All strengths
  • Quadrivalent Meningococcal Polysaccharide Vaccine (Menomune-A/C/Y/W-135) – 0.5 mL vial
  • Sodium Bicarbonate 4.2% – 10 mL syringe & vials
  • Sodium Bicarbonate 7.5% – 50 mL syringe
  • Sodium Bicarbonate 8.4% – 10 mL syringe, 50 mL syringe, & 50 mL vials
  • Sodium Chloride 23.4% – 30 mL vials
  • Tigecycline 50 mg/10 mL vials
