As you know, Pennsylvania recently enacted the CARE (caregiver advise, record, enable) Act, to improve caregiver involvement in care coordination and discharge planning. Some studies have shown that if we involve patients’ caregivers in care coordination effectively, hospital readmissions could be reduced by as much as 25%! Because it is a new program, we may have some opportunities related to caregiver involvement. I am performing a pilot quality improvement project at Paoli Hospital as my doctoral project, consisting of concurrent surveys between caregivers of patients, and hospital personnel, followed by a gap analysis, to determine how well we are doing with this initiative.
