Mandatory Vaccination at Main Line Health

  • Main Line Health’s commitment to patient safety led to a new policy to keep patients safe and employees healthy during each flu season.   Influenza vaccine is available free of charge to all MLHS employees, students, volunteers, members of the medical staff, members of the specified health professional staff and contracted clinical personnel.
  • Deadline for compliance to the mandatory influenza program is Dec 5, 2016.
  • Prior to receiving your vaccination, employees should complete the electronic My MLH Flu Consent Form located on the intranet in MLH Self Service.  If you do not have access to MLH Self Service/My MLH Flu Consent Form, you will complete a paper consent form at the time of your vaccination.
  • An MLH badge is required at the time of vaccination.
  • shoulderdiagram1Please remember to dress appropriately to receive the vaccine safely per the diagram at right. If the nurse cannot access the safe area, you will be asked to come back in more appropriate attire.
  • MLH now has egg-free influenza vaccine. If you previously were exempt from vaccination due to egg allergy, you are now required to comply with the MLH mandatory influenza vaccination policy, as all egg allergy medical exemptions have been withdrawn. Those with egg allergies should contact MLH Occupational Health at 484.565.1293 to schedule vaccination.
  • Those who wish to apply for a medical exemption for the 2016-2017 year should use the Flu Medical Exemption Form.

Staff who may be vaccinated outside Main Line Health should submit proof of vaccination (provider’s note or receipt listing flu vaccination, marked with employee’s name and ID number) in one of the ways listed below. Please retain a copy for your personal records.
